First-Time Patients

Thank you for considering our practice. We are currently accepting new patients. All new patients will be seen in the office; no exceptions. Please contact us by phone or by completing the Contact form, to make an appointment.

If you are a new patient and would like to know if we accept your specific insurance plan, please complete the Contact form, fill in the required information and upload an image of the front and back of your insurance card. A member of our staff will contact you to schedule an appointment. If we do not accept your health insurance plan, someone will call to advise you.

CHECKLIST: bring to YOUR first visit

  1. Current insurance card

  2. Valid photo ID, e.g. driver’s license

  3. Visa or Mastercard to secure on file

  4. Cash, Visa or Mastercard for office co-pay due at time of service

  5. Recent lab/radiology results from your referring doctor

  6. Valid insurance referral (if your plan requires one)

  7. Printed copy of New Patient Packet.

Existing Patients

All follow-up appointments will be in the office.

If you are an existing patient and need to send lab results or glucose logs, you can upload them to the Contact form and the documents will be forwarded to the doctor. Please note that the form is sent to a secure e-mail viewable only by the Office Manager and the lead medical assistant. Please do not send any medical questions that you need answered by the doctor through this form.

Patient Portal

Use the Patient Portal and log into a secure online account to view test results, request prescription refills, pay bills, e-mail securely with the office, and more.
